Item and aug eaten

Grubb used some Lillus Magical Distillers to remove a stone of defense augment from a mask of venom. When i did that it took all three and in return gave me a Shissar Empire Badge of Honor which then deleted as i already had one on my person at the time. Please look into this for me. Thanks. Obviously i cannot recreate this lol.
