This item drops from Wall of Slaughter from the feran mother boss. The ingame thumbnail is the two-pronged claw (h2h), however, the graphic appears as a bag.

I noticed it too and also a few other items from WoS have kinda a plain graphic for the Tier , however on the other hand how long you really gonna keep it ?
