What NPC gives the instant in The Hidden Vale that drops augments like: Icon of the Fallen Queen?

Check the NPC back in Vale (near the Token NPC).
There are two instances: Enomuth and Fallen Queen.

You have to complete the Enomuth mission before you can do the Queen; they are sequential.

yes, you can repeat them. You also get the task when you re-enter after completing so I assume you can get the aug(s) again if you need to put it in other gear and have no distillers, but I have not had to move mine yet so the devs or someone that has done so would need to confirm.

Yes, if you destroy the aug and complete the mission again you will receive a new one, shinny and pretty :p

What AC does a tank need to make it "safely"?
