1.5 stone issue

Hi, I've completed 1.5 but can't find my stone of the reckless for the turn in. I've looked in every bag I have with no luck. Thinking I could get a replacement I killed the 4 mobs again for the eyes but Fangor simply looks confused and gives them back. Could a GM please tell me what I need to do to correct my blonde moment?
