I experienced a similar problem and was able to chalk it up to MQ2. My SK had a mq2melee setting that was clicking an Anger item (they were gauntlets that needed to be equipped, so the routine would equip/click/bag the gaunts) which was interfering. I found that this issue occurred randomly: sometimes it would be days without issue, other times it would be back to back encounters. Since it was so random, I had to assume it was something else that was a little "random." The solution? I removed the Anger clicky from my mq2melee settings and I've not experienced the problem since.
I have also noticed a similar situation on casters: Mage has a routine to cast swarm pets on a trigger, but if I trigger the routine while he's casting/as he begins to cast, it will try to use the AA but see a spell is/about to cast and uses the AA timer without getting the affect. The solution here was to build in a little "safety routine" so that no spells except for the swarm pets could be cast after triggering the swarm pet routine.
Hopefully you're a MQ2 user and this makes sense (mq2melee/mq2cast are the culprits!). If not, hopefully a MQ2 user who experiences similar problems finds this post in the future.
