EZ Server has a particular quest they call their "Prestige Quest" which I believe was stolen from Ultima Online, or some other MMO. The concept is simple, though. Quest NPC offers you a reward at a starting level. This reward could be any stat item (EZ uses a ring, for example). After accepting the quest, you are stripped of all buffs and pets, and are deleveled to level 1. NPC won't allow you to accept unless you are naked, to avoid exploiting high end gear to aid in leveling. You are then required to level back to 70. Once you reach level 70, you may hand in your reward item to receive the "next level" item, and you are deleveled back to 1 to repeat the process. Each time you complete the quest and are deleveled back to 1, your reward gains in strength. Since the reward is equippable at level 1, you can use it to aid in leveling back up. The higher in levels the item becomes, the easier it becomes to level (adding valuable stats, like ac, hp, and regen). You can always choose to level back to 70, but not turn in the reward item to receive the upgrade to avoid deleveling again.
With this concept, the options are many. The reward(s) could be a non-visible slot item, or a weapon, augment, etc. You can make the increments as small or as large for the next level upgrade as you so choose. I think it's a great way to create a timesink. I would just avoid the huge problem EZ server has created with this reward, where it is nearly impossible to complete the highest tiers of their content without completing atleast up to level 50 of their prestige quest. (Yes, that means leveling 50 times over!) The end result (level 100) is some 50k hp ring, or some sort, which is necessary to live through the end content.
Also, with the inevitable powerleveling to complete this quest multiple times, proper measures would need to be in place to avoid the issues with overcrowding in lower end custom zones, pulling large trains to die on damage shields, and exploiting buff npcs, etc. But it would be a fun way to give people with too much time on their something to do for a little reward that is repeatable!
